ghrelin and obesity

The precise role of ghrelin in the pathophysiology of obesity is still under investigation. The journal Obesity Reviews recently published an article evaluating the effects of combined exercise, sleep and stress management on hormones. Ghrelin and Obesity — A story of Moderation and the Mediterranean Diet. Ghrelin is a hormone associated with feeding and energy balance. It’s considered by some that if we’re able to get a firm grip on how ghrelin initiates appetite, then increasing its level could be a revolutionary new method of obesity management and treatment. Ghrelin levels in the plasma of obese individuals are lower than those in leaner individuals, suggesting that ghrelin does not contribute to obesity, except in the cases of Prader-Willi syndrome-induced obesity, where high ghrelin levels are correlated with increased food intake. Plasma ghrelin levels are low in human obesity, and are generally associated with increased weight.
Ghrelin mRNA expression in the gastric fundi of db/db mice, an obese model characterized by a null mutation in the leptin receptor gene ( 25 , 26 ), is down-regulated compared with control mice ( 18 ). Ghrelin, obesity & appetite. The only known overweight condition associated with high ghrelin levels is the Prader-Willi syndrome. Not surprisingly, this hormone is secreted in response to acute stressors and it is chronically elevated after exposure to chronic stress in tandem with a number of metabolic changes aimed at attaining homeostatic balance. ... Ghrelin is a key player in metabolism and it seems to be determined by eating behaviors during pregnancy. 83 The most common form of human syndromic obesity, Prader-Willi syndrome, results from defective expression of several imprinted genes on chromosome 15.
In obesity, individuals develop resistance to leptin and ghrelin, which have important functions in the neuroendocrine control of energy homeostasis. Ghrelin was discovered for its ability to bind the growth hormone secretagogue receptor (GHSR1a) and stimulate growth hormone release. However, much research conducted with this novel stomach hormone is focused on proposed roles for it to participate in regulating energy balance. Researchers from the study concluded that “Behavioral interventions could offer a practical, cost-effective method for reducing or stabilizing ghrelin levels after initial weight loss to improve weight loss maintenance.” ( 15 )
